Test Requisition Form (TRF).
If you’re not registered on the Naveris Provider Portal, you can still place a NavDx test order by downloading and completing a paper Test Requisition Form (TRF). With the standard TRF, you can order a NavDx test for any patient. Complete all required fields, obtain the necessary signatures, and submit the form with the collected specimen.

Blood Collection Options.
NavDx testing requires a standard blood draw. We offer flexible phlebotomy options to accommodate your practice workflow and patient needs.
In-Office Collection
Order NavDx test kits for delivery directly to your practice or facility. Your phlebotomy staff performs the blood draw using the provided collection kit and ships the sample to the Naveris laboratory.
NavDx-Arranged Phlebotomy
For patients unable to come to your practice, Naveris can arrange for convenient blood collection at the patient’s home, workplace, or at a walk-in facility near the patient. Learn more→
What to Expect
After Ordering.
Once your kit is received, the specimen collected and shipped, and the sample arrives at the Naveris laboratory, your NavDx results are typically available within 7 business days. You will receive notification through the Provider Portal when results are ready.
Each NavDx report includes:
- TTMV-HPV DNA result (positive, negative, or indeterminate) and Score (if positive)
- Results summary by HPV genotype with Risk Score Interpretation
- Longitudinal timeline showing current and all prior test results for that patient
- Interpretive Comments: A board-certified Clinical Pathologist reviews every report and provides written interpretive comments. Each report is quality-controlled by a clinical laboratory specialist before Medical Director review and release.
